Keeping infants and toddlers clean, dry, and healthy is key to building a solid foundation for all children to reach their full potential. But one in three families struggles to provide clean diapers for their baby. Many families in our community often must decide between buying food or diapers for their baby.

Our comprehensive services allow you to receive all needed dental care right here in our state-of-art office – from dental cleanings and fillings to extractions. Even if it’s been years since you last visited the dentist, we can help. Our comfortable office, compassionate team, and minimally-invasive treatments will help you feel completely at ease.
Primary Care
Our Family Practice Providers are here to be a partner in your healthcare journey! We offer comprehensive healthcare services including annual exams, medication and chronic disease management, vaccinations, lab services, same day and walk-in appointments for illness and injury.
Behavioral Health
We believe that physical health is affected by a patient’s emotional and behavioral health and both aspects are important to your well-being. We are the community’s partner for providing medications for drug and alcohol addiction. Treatment works and recovery is possible! Our recovery program includes assessments, medication, private consultations, therapy, and peer support.
Emergency Dental Services
Our Urgent Dental Clinic is open for WALK-IN dental emergencies Monday thru Friday from 9 a.m. – 11 a.m. and 12:30 p.m. to 3:00 p.m.. Dental emergencies include dental pain, toothaches, or dental infection.
Pediatrics
We are dedicated to partner with you to ensure your children are healthy and happy! We see all ages - infants, children and teens. At our clinic your child can see a dentist and a medical provider all at one place! We are here to ensure your children receive quality dental preventive care, well-child exams, sports physicals, immunizations, injuries and illness.
Pediatric and Adult Oral Surgery
Our surgery team performs procedures for patients of all ages that require IV sedation or general anesthesia. We are sensitive to the patient (and parent!) needs and look forward to providing a stress-free wisdom teeth removal experience!
Patient Centered Medical Home
Comprehensive care management through care coordination, health promotion, comprehensive transitional care including follow-up from inpatient, ER, other settings, patient and family support, and referrals to community and support services.
Walk-In Medical Care
Rolla Family Clinic has joined forces with Four Rivers and moved the walk-in clinic to 1081 E. 18th Street in Rolla. With over 20 years of experience, our acute care-trained team is prepared to care for your medical needs, making it an affordable alternative to the ER.
Smile Bus
To keep those smiles shining children see a dentist twice a year! Our dental team takes pride in creating beautiful smiles for our local area students. The mobile dental unit provides school-based care at the school giving students access to dental care that they might not be able to receive.
At Four Rivers, we are recognized by NCQA as a Patient-Centered Medical Home (PCMH). What does that mean for our patients? Simply put, we are able to provide a model of evidence-based patient care that puts the PATIENT at the forefront of their healthcare. By building relationships between the patient, our providers, clinical staff, and their outside care team members, we are able to improve the continuity of care and overall quality of life of those we serve.
We provide Health Home and Care Management services to patients at Four Rivers to better manage their chronic health conditions (including diabetes, hypertension, COPD, obesity, anxiety, and more), reduce unnecessary hospitalizations and ED visits, improve patient experiences, and lower overall healthcare costs.
